A Unix type account at an ISP that supports the MySQL database engine and allows you cgi-bin access.
The Mysqlperl module or the DBI/DBD modules that allow Perl to talk to the Mysql database.
A digital certificate for your domain.
Some familiarity with installing Perl scripts.
